Refferal Service

We believe that accurate placement matching is essential to make sure that we are able to meet the  needs of vunerable adults . We make no apologies for requesting detailed information (where available) from placing social workers and look throughout the Assessment period to seek further information from family members.

Referall Recieved

1. Asess wether person meets home criteria

2. Requested any further information required

3. Registered manager completes referral process as outlined in policy

Meet With The Resident

1.The manager and social worker meet with the resident and explain the ethos of the home and share the guide of the home.

2. Decision made about suitability of placement with matching of placement.

Resident Views The Home.

1. Resident , views the home.

2.Resident, Social worker and Registered Manager in agreement for placement to begin with inital assesment.

3.Manager ensures all necessary paperwork is in place, including risk assesment.

Resident  Moves In

1.Resident is supported through a transition period to move into home.
